Coordinated Pathways for Youth

Our Collaborative Youth Wellbeing Framework has 4 key components or types of support.

  1. A community-based 24/7 Youth Support Centre with mental and physical health clinics and up to 12 beds) where the initial intake and baseline holistic assessment processes take place. These Youth Support Centres are intended to replace the traditional model of a overnight youth shelter, where youth in crisis are required to travel out of their communities, where there are large numbers of youth or older adults, with very little staff support, no mental health or physical health services, and many barriers.
  2. Specialized step up and step down youth supportive and transitional housing and complex care organizations who continue assessing the development of youth and oversee their care and skill building.
  3. The community-based skill developing organizations that allow each youth to achieve physical, emotional, mental and spiritual/cultural well-being based on their individual needs.
  4. On the ground and drop-in outreach youth agencies that support youth in community as they practice independence, identity youth in crisis requiring complex care, support basic and survival needs, and provide ongoing relationships and support.

Our framework integrates these 4 components with process pathways and data sharing to ensure a true continuum of complex care.

We do this with the help of the following tools:

The MyCompass Data Platform

To ensure youth don’t have to retell their story at every door and that we are prepared to support them when they arrive. For the first time, we are able to share stories, data, and outcomes for young people with complex needs.

Youth Holistic Wellbeing Assessments

We assess and support youth holistically and collectively measure outcomes individually based on their journeys. The assessments were built by youth workers based on indigenous ways of knowing and being.

Youth Pathways and Process Maps

Platforms and models are only as good as the relationships and practices between agencies and individuals. We continue to meet regularly to improve and sustain these relationships and pathways.
